Name*: User's full name
Company: User's Company Name
Email: User's Email Address
City: User's City Name
State/Province: User's State or Province Name
Postal Code: User's Postal Code (can be 5 digit code or 9 digit code with dash)
Country: User's Country Name
Phone: User's Phone Number where they can be contacted
